Maintenance Requests
To submit standard maintenance requests please utilize the link below; all emergency requests MUST be called in to the Engineering office at extension 4220 (M-F 8am – 4:30pm) or the House Administrative Nurse (HAN) at extension 4199 (all after hours/weekends).

Instructions on Submitting a Maintenance Request

1. Click the link to submit a Service Maintenance Request.
2. The Welcome page gives basic instructions.
3. Please provide your name, e-mail address, and extension
4. Please Drill down using the pull down menu to find the location for which you want to report a problem. Drill down as far as possible.
5. Please Select a problem. There are a few generic problems if your specific problem is not listed, such as equipment broken and facility damage.
6. The problem you selected will automatically copy to the short description field.
7. Add specific details describing the problem.

If there are any questions or you are having difficulties completing this request, please utilize the HELP link on the left hand side of the Service Request or call Engineering at extension 4220.
