Interactive Screening Program

Life can be stressful at times. Whatever you’re struggling with – family pressures, work burnout, relationship issues, grief, stress or financial concerns – it is easy to get overwhelmed without an idea of where to turn for support. Please know that you are not alone and that support is available. Natividad hospital staff are invited to complete this brief anonymous questionnaire to get confidential feedback and support from an EAP counselor. The EAP counselor provides information and support for connecting to available mental health services.

Take 3 easy steps:

  1. Click the “Sign-up” button below
  2. Create a User ID and Password, allowing for complete anonymity
  3. Take and submit a brief (<10 minute) questionnaire

A counselor will review your questionnaire and post a personal response to you on the secure website. The response will include information, recommendations, and optional next steps. You decide what’s next. You will have the options of communicating with the counselor through the website, on the phone or in person. Or, you can decide to do nothing further at this point.

Protecting your Privacy

Your identity will not be known to the counselor unless you decided to share it. No one at Natividad has access to your records. It is completely confidential and anonymous.

OSHA: Healthcare Workplace Violence Prevention

Workplace Violence Prevention has been a focus of the healthcare community for many years. The California Legislature first entered the debate in 1993 with the passage of AB 508 (Chapter 936, Stats. 1993). This bill enacted Health & Safety Code Section 1257.7 and 1257.8 requiring hospitals to conduct annual security & safety assessments & implement a security plan to protect employees, patients & visitors from aggressive & violent behavior at work.

These two laws also require hospitals to report injuries sustained by on-duty hospital personnel to law enforcement, and to provide training to hospital employees regularly assigned to the emergency department and other high-risk areas, as identified by the hospital. In 2014, the Legislature again passed legislation regarding hospital Workplace Violence Prevention. SB 1299 (Chapter 842, Stats. 2014) required the CAL/OSHA Standards Board to adopt a regulation requiring hospitals to include a Workplace Violence Prevention Plan as part of the Injury & Illness Prevention Plan (IIPP) that all employers are required to establish.

  • If you have been involved in an incident with an individual(s) while at work, please immediately report the incident to your supervisor and/or House Administrative Nurse to complete necessary Workers Compensation Forms (required to seek treatment) AND complete the form below, titled  Upon completion of the form WPV Data Collection Tool, please click the “submit” button. The hospital’s Safety Investigation Coordinator, will be notified & will contact you regarding the incident.

To find the current SDS click the link below, you will be taken to a screen that will allow you to search for the SDS you are looking for by product or by manufacturer. You can also sort all by the letter that the chemical starts with.

Workers’ Compensation

Intercare Holdings Insurance Services currently provides administration of all workers compensation claims on behalf of The County of Monterey. In the event you are injured at work, please immediately report this injury to your manager or House Supervisor. All appropriate documentation (DWC-1, Form 5020) should be submitted to the benefits coordinator so they can effectively manage the claims. Please do not call claims in directly to the carrier.
To ensure speedy & efficient processing of your work related injury, please complete & submit the packet below to the Benefits Coordinator. Please note, cannot be initiated until these documents are received.

AAll reports of Non-Employee incidents should be recorded on the Incident Report form and provided to the benefits coordinator in HR for record keeping purposes.
This incident should also be reported to the staff members hiring agency (i.e. , Staffing Agency, Registry, etc.) as they may require additional forms to be completed

Disaster Preparedness

All staff are required to respond in an emergency.  Natividad employees have an additional responsibility as some patients are incapable of self-preservation.  To be prepared for an emergency, all employees are required to be familiar with the following codes:

Fire Response
Natividad uses the RACE acronym (described below) as the immediate response to a fire:

Rescue – Remove individuals and patients in immediate danger of the fire.

Alarm – Alert other staff of the fire by yelling “CODE RED” and the location. If time allows, activate the nearest manual fire alarm station. Call the operator by dialing 66 and provide the necessary information. If time does not permit, have other staff activate the alarm and call the operator.

Confine – Confine the fire to the room in which it is in by closing the door and windows, if possible. Prevent the fire from spreading by closing all doors in the area.  Do not re-enter the room after it has been evacuated and the door has been closed.

Evacuate – Prepare for evacuation by clearing all carts and other obstacles from corridors that may impede relocating people to an area of refuge   Evacuate patients and staff if so advised by fire personnel or administration.
If the fire is in an incipient stage, and if it is safe to do so, the fire can be extinguished using a fire extinguisher.  The acronym PASS describes how to properly use a portable fire extinguisher:

Ergonomics

To prevent repetitive motion injuries at your computer workstation, verify that your habits and setup are ergonomically correct.

Natividad is now equipped to provide an in house on-site ergonomic evaluation to help you identify changes to make your workstation for ergonomic from one of our certified ergonomics assessment specialist.

Prior to requesting an evaluation, make sure you complete the following:

 

Often, simple changes like adjusting the height of your seat or monitor can help you sit more ergonomically. Once you have done this and adjusted your workstation and you still wish to have an ergonomic evaluation you may submit a request by sending an e-mail to this address (ergo@natividad.com). Make sure to include your availability, supervisor, office location, your Healthstream certificate of completion for the ergonomics training, include in your e-mail that you have read and understand the office ergonomics guide, and any additionally information we should be made aware of.

The Ergo Desk also offers on-site ergonomic evaluations from contracted Ergonomist.

Make your request by submitting the online Request Form below. The Ergonomic Evaluation Request Form will need to be filled out electronically and submitted to your supervisor via email.  The supervisor will submit the approved request to the Ergo Desk via email. Please allow up to 7 days for your request to process from the date your supervisor submits to the Ergo Desk. You will then receive an email and be asked to complete an ergonomics self-assessment of your work area with instructions.  When all required documents have been completed (Ergo request form, self-assessment part 1 and 2) and received by the Ergo Desk, you will be scheduled in the order received.

Following the evaluation:

  • You receive a notice of completion of your report and arrange to meet with your Ergonomic Coordinator to discuss and sign off on the report and recommendations.
  • Products are recommended for purchase in some cases. Note: No one product will guarantee proper ergonomics or necessarily be an improvement for every individual or situation. Proper selection, adjustment, and use of equipment are essential practices for reducing the risk factors commonly associated with repetitive motion injuries.
  • Register for the appropriate training(s).

Following receipt of your products (if recommended):

  • Read instructions or use online website instructions regarding equipment use
  • Set up for correct use
